Aleksey Grichenko has composed a "reference" chapter on the NCBI Data Model:

It's mostly a structured wrapper that contains references to the Data Model
chapters from the NCBI C Toolkit documentation (written by Jim Ostell) but
also provides some ties to the relevant C++ Toolkit classes and APIs.
