id="_isovaleric-a_Summary_">Summary</h2><div><h4 class="inline">Clinical characteristics.</h4><p>Individuals with clinical manifestations of isovaleric acidemia (IVA) have either classic IVA identified on newborn screening or classic IVA with a later diagnosis due to a missed diagnosis or later onset of clinical manifestations. Classic IVA is characterized by acute metabolic decompensations (vomiting, poor feeding, lethargy, hypotonia, seizures, and a distinct odor of sweaty feet). Acute metabolic decompensations are typically triggered by fasting, (febrile) illness (especially gastroenteritis), or increased protein intake. Clinical deterioration often occurs within hours to days after birth. Additional manifestations of classic IVA include developmental delay, intellectual disability and/or impaired cognition, epilepsy, and movement disorder (tremor, dysmetria, extrapyramidal movements). Early treatment in those identified by newborn screening can significantly reduce morbidity and mortality in individuals with classic IVA.</p></div><div><h4 class="inline">Diagnosis/testing.</h4><p>The diagnosis of classic IVA is established in a proband by identification of C5-carnitine metabolites by tandem mass spectrometry and isovalerylglycine (IVG) and 3-hydroxyisovaleric acid (3-HIVA) on analysis of urinary organic acids by gas chromatography-mass spectrometry, or identification of biallelic pathogenic variants in <i>IVD</i> by molecular genetic testing.</p></div><div><h4 class="inline">Management.</h4><p><i>Targeted therapy:</i> Low-leucine/protein-reduced diet and the supplementation of a leucine-free formula in infants or leucine-free amino acid mixture in older children; carnitine and/or glycine supplementation.</p><p><i>Supportive care:</i> Routine daily treatment includes education of affected individuals and caregivers about the natural history, maintenance and emergency treatment, prognosis, and risks of acute encephalopathic crises; emergency treatment letter and MedicAlert<sup>&#x000ae;</sup>; management of movement disorder per neurologist; physical therapy and aggressive rehabilitation therapy for gross motor delay; notify metabolic center prior to planned surgeries; consult metabolic disease specialist with any emergency surgery/procedure.</p><p>Emergency outpatient treatment includes carbohydrate supplementation orally or via tube feeding, transient reduction of natural protein intake, elevation of carnitine supplementation, and glycine; antipyretics for fever; antiemetics for vomiting.</p><p>Acute inpatient treatment includes stopping protein intake, intravenous glucose, and hydration with normal saline; adjusting treatments for new or evolving neurologic manifestations; consider buffers as needed for life-threatening metabolic acidosis; nitrogen scavengers for hyperammonemia.</p><p><i>Surveillance:</i> Quantitative analysis of plasma amino acids at least every three months until age one year, every six months from age one to six years, and annually in those age six years and older; blood gases, albumin, calcium, phosphate, parathyroid hormone, complete blood count, and vitamin B<sub>12</sub> at least annually in those on a protein-restricted diet; measurement of growth and head circumference at each visit throughout childhood; monitor weight throughout adulthood; monitor developmental milestones at each visit; neuropsychological testing and standardized quality-of-life assessments as needed; assessment of movement disorder at each visit.</p><p><i>Agents/circumstances to avoid</i>: Excess of dietary protein or protein malnutrition inducing catabolic state; prolonged fasting; catabolism during illness.</p><p><i>Evaluation of relatives at risk:</i> Biochemical or molecular genetic testing of all at-risk sibs of any age is warranted to allow for early diagnosis and treatment of classic IVA.</p></div><div><h4 class="inline">Genetic counseling.</h4><p>Classic IVA is inherited in an autosomal recessive manner. If both parents are known to be heterozygous for an <i>IVD</i> pathogenic variant, each sib of an affected individual has at conception a 25% chance of being affected, a 50% chance of being an asymptomatic carrier, and a 25% chance of being unaffected and not a carrier. Once the <i>IVD</i> pathogenic variants have been identified in an affected family member, carrier testing for at-risk relatives and prenatal and preimplantation genetic testing are possible.</p></div></div><div id="isovaleric-a.Diagnosis"><h2 id="_isovaleric-a_Diagnosis_">Diagnosis</h2><div id="isovaleric-a.Suggestive_Findings"><h3>Suggestive Findings</h3><div id="isovaleric-a.Scenario_1_Abnormal_Newborn"><h4>Scenario 1: Abnormal Newborn Screening (NBS) Result</h4><p>NBS for classic isovaleric acidemia (IVA) is based on the quantification of the first-tier analyte isovalerylcarnitine (as C5-carnitine) in dried blood spots.</p><p>C5-carnitine (C5) values above the cutoff reported by the screening laboratory are considered positive and suggest a diagnosis of classic IVA; additional testing of urinary organic acids is required to <a href="#isovaleric-a.Establishing_the_Diagnosis">establish the diagnosis</a>. Such testing can be performed in regions with increased use of pivalic acid-containing antibiotics (taken by mother and child) and pivalic acid-containing skin cream derived from pivaloylcarnitine. Analysis of urinary organic acids (see <a href="#isovaleric-a.Establishing_the_Diagnosis">Establishing the Diagnosis</a>) can also exclude false positive results on tandem mass spectrometry due to pivaloylcarnitine, or due to unrelated metabolites secondary to 2-methylbutyrylglycinuria [<a class="bibr" href="#isovaleric-a.REF1" rid="isovaleric-a.REF1">AWMF 2019</a>] (see <a href="#isovaleric-a.Differential_Diagnosis">Differential Diagnosis</a>).</p><p>The majority of individuals with classic IVA present acutely with life-threatening metabolic decompensation during the first two weeks of life. Therefore, infants with C5 &#x0003e;4 &#x000b5;mol/L on the first NBS sample that do not have access to C5 isobar analysis should begin the following medical interventions while urine organic acids are performed to establish the diagnosis of classic IVA [<a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2023b.1063" rid="isovaleric-a.REF.m_tze.2023b.1063">M&#x000fc;tze et al 2023b</a>]:</p><ul><li class="half_rhythm"><div>Promotion of catabolism and avoidance of fasting</div></li><li class="half_rhythm"><div>Clinical assessment by a pediatrician or at a pediatric metabolic center including measurement of ammonia and blood gases</div></li><li class="half_rhythm"><div>Consideration of carnitine supplementation (50-100 mg/kg/day orally or intravenously)</div></li></ul></div><div id="isovaleric-a.Scenario_2_Symptomatic_Indi"><h4>Scenario 2: Symptomatic Individual</h4><p>A symptomatic individual can have delayed diagnosis of classic IVA or untreated classic IVA due to any of the following: NBS not performed, false negative NBS result, or caregivers not adherent to recommended treatment following a positive NBS result. Absence of a known family history does not preclude the diagnosis.</p></div></div><div id="isovaleric-a.Establishing_the_Diagnosis"><h3>Establishing the Diagnosis</h3><p>The diagnosis of classic IVA in a proband with <a href="#isovaleric-a.Suggestive_Findings">suggestive</a> biochemical and/or clinical findings <b>is established</b> by identification of:</p><ul><li class="half_rhythm"><div class="half_rhythm">C5-carnitine (C5) metabolites, isovalerylglycine (IVG), and 3-hydroxyisovaleric acid (3-HIVA) on analysis of urinary organic acids by gas chromatography-mass spectrometry [<a class="bibr" href="#isovaleric-a.REF.tavares_de_almeida.2022" rid="isovaleric-a.REF.tavares_de_almeida.2022">Tavares de Almeida &#x00026; Ribes 2022</a>];</div><div class="half_rhythm">

Of infants identified on NBS in Germany, 80% had attenuated IVA. Biochemical findings (e.g., only mild elevations in metabolites) may be helpful to distinguish classic IVA from attenuated IVA [<a class="bibr" href="#isovaleric-a.REF.m_tze.2023b.1063" rid="isovaleric-a.REF.m_tze.2023b.1063">M&#x000fc;tze et al 2023b</a>]. (2) Per ACMG/AMP variant interpretation guidelines, the terms "pathogenic variant" and "likely pathogenic variant" are synonymous in a clinical setting, meaning that both are considered diagnostic and can be used for clinical decision making [<a class="bibr" href="#isovaleric-a.REF.richards.2015.405" rid="isovaleric-a.REF.richards.2015.405">Richards et al 2015</a>]. Reference to "pathogenic variants" in this <i>GeneReview</i> is understood to include any likely pathogenic variants. (3) Identification of biallelic <i>IVD</i> variants of uncertain significance (or of one known <i>IVD</i> pathogenic variant and one <i>IVD</i> variant of uncertain significance) does not establish or rule out the diagnosis.</div></li></ul><div id="isovaleric-a.Molecular_Genetic_Testing"><h4>Molecular Genetic Testing</h4><p><b>Single-gene testing.</b> Sequence analysis of <i>IVD</i> is performed first to detect missense, nonsense, and splice site variants and small intragenic deletions/insertions. Recurrent decompensations are less frequent in individuals identified through NBS compared to those diagnosed based on symptoms alone, and the frequency of decompensations tends to decrease with age, with no instances reported after early adolescence [<a class="bibr" href="#isovaleric-a.REF.gr_nert.2012.9" rid="isovaleric-a.REF.gr_nert.2012.9">Gr&#x000fc;nert et al 2012</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>].</div></li><li class="half_rhythm"><div class="half_rhythm">Individuals with <b>classic IVA not identified by NBS</b> usually develop the first manifestations of acute metabolic decompensation after a short symptom-free period after birth. Clinical deterioration often occurs within hours to days after birth with feeding refusal, recurrent vomiting, progressive weight loss, generalized hypotonia, abnormal posturing, and abnormal movements, and a distinctive odor of sweaty feet can be present. Laboratory abnormalities include severe metabolic acidosis with elevated anion gap, elevated lactate, ketosis, and hyperammonemia [<a class="bibr" href="#isovaleric-a.REF.dionisivici.2006.383" rid="isovaleric-a.REF.dionisivici.2006.383">Dionisi-Vici et al 2006</a>, <a class="bibr" href="#isovaleric-a.REF.k_lker.2015a.1041" rid="isovaleric-a.REF.k_lker.2015a.1041">K&#x000f6;lker et al 2015a</a>, <a class="bibr" href="#isovaleric-a.REF.k_lker.2015b.1059" rid="isovaleric-a.REF.k_lker.2015b.1059">K&#x000f6;lker et al 2015b</a>]. Subsequently, individuals may exhibit lethargy, seizures, and coma, leading to death within a few days or resulting in severe neurologic damage if diagnosis of classic IVA is not identified and/or treatment is not started.</div><div class="half_rhythm">Metabolic decompensations can occur any time throughout childhood in individuals with classic IVA (regardless of age of diagnosis). Metabolic decompensations did not occur after early adolescence in 21 children with classic IVA [<a class="bibr" href="#isovaleric-a.REF.gr_nert.2012.9" rid="isovaleric-a.REF.gr_nert.2012.9">Gr&#x000fc;nert et al 2012</a>]. However, data on the proportion of individuals affected, age of onset, seizure types, seizure frequency, and EEG findings are limited. One child, age five months, was described with infantile spasms and hypsarrhythmia observed on EEG during a metabolic decompensation [<a class="bibr" href="#isovaleric-a.REF.sezer.2016.376" rid="isovaleric-a.REF.sezer.2016.376">Sezer &#x00026; Balci 2016</a>].</p><p><b>Developmental delay.</b> Delays in gross and fine motor development and speech and language have been reported in individuals with classic IVA detected through NBS, particularly those with neonatal metabolic decompensation despite early diagnosis by NBS [<a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>]. However, in individuals with classic IVA diagnosed after the onset of symptoms, developmental disorders affecting motor and cognitive functions are more prevalent, particularly in those with a late diagnosis (&#x0003e;50% had developmental delay) [<a class="bibr" href="#isovaleric-a.REF.gr_nert.2012.9" rid="isovaleric-a.REF.gr_nert.2012.9">Gr&#x000fc;nert et al 2012</a>]. Delayed motor development is often accompanied by muscular hypotonia and movement disorders.</p><p><b>Intellectual disability&#x000a0;/ impaired cognition.</b> In one cohort, individuals with classic IVA identified on NBS had lower IQ levels than the general population (mean IQ: 91&#x000b1;10), and IQ levels were even lower in those who had experienced severe neonatal metabolic decompensation [<a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>]. In another cohort of 16 affected individuals with classic IVA, cognitive function was within the normal range for those identified through NBS and clinically diagnosed individuals [<a class="bibr" href="#isovaleric-a.REF.couce.2017.355" rid="isovaleric-a.REF.couce.2017.355">Couce et al 2017</a>].</p><p>Among individuals with a later diagnosis, approximately 60% were reported to have normal neurocognitive outcomes. Learning disabilities were observed in approximately one quarter of affected individuals. Severe cognitive dysfunction was reported in 5% of individuals.</p><p>Early diagnosis and treatment in those who survive the initial metabolic decompensation tends to result in better neurocognitive outcomes [<a class="bibr" href="#isovaleric-a.REF.gr_nert.2012.9" rid="isovaleric-a.REF.gr_nert.2012.9">Gr&#x000fc;nert et al 2012</a>]. A multicenter study showed that early diagnosis and treatment significantly impact the clinical and neurocognitive outcomes of individuals with classic IVA [<a class="bibr" href="#isovaleric-a.REF.heringer.2016.341" rid="isovaleric-a.REF.heringer.2016.341">Heringer et al 2016</a>].</p><p><b>Movement disorder</b> (tremor, dysmetria, extrapyramidal movements). The onset of a movement disorder usually occurs early in the disease course, particularly in infants and children with recurrent acute metabolic decompensations prior to diagnosis and the start of treatment. Depending on the extent of brain damage, the severity of the movement disorder can vary; however, it is mostly mild to moderate. Since the movement disorder is the neurologic sequelae of preceding brain damage, the movement disorder is stable unless brain damage is aggravated through recurrent severe metabolic decompensations.</p><p><b>Behavioral problems.</b> No behavioral abnormalities have been reported in cohorts of individuals with classic IVA identified through NBS [<a class="bibr" href="#isovaleric-a.REF.heringer.2016.341" rid="isovaleric-a.REF.heringer.2016.341">Heringer et al 2016</a>, <a class="bibr" href="#isovaleric-a.REF.schlune.2018.7" rid="isovaleric-a.REF.schlune.2018.7">Schlune et al 2018</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>]. Although data is limited, behavioral problems such as attention-deficit/hyperactivity disorder have been reported in individuals with classic IVA not identified on NBS [<a class="bibr" href="#isovaleric-a.REF.hertecant.2012.671" rid="isovaleric-a.REF.hertecant.2012.671">Hertecant et al 2012</a>]. These issues may arise during episodes of metabolic derangement or due to the impact on the central nervous system.</p><p><b>Hematologic manifestations</b> have not been reported in individuals with classic IVA detected through NBS [<a class="bibr" href="#isovaleric-a.REF.vockley.2006.95" rid="isovaleric-a.REF.vockley.2006.95">Vockley &#x00026; Ensenauer 2006</a>, <a class="bibr" href="#isovaleric-a.REF.schlune.2018.7" rid="isovaleric-a.REF.schlune.2018.7">Schlune et al 2018</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>], unlike in other forms of organic acidemias, particularly <a href="/books/n/gene/mma/?report=reader">methymalonic acidemia</a> and <a href="/books/n/gene/propionic-a/?report=reader">propionic acidemia</a>. Pancytopenia as well as isolated neutropenia, thrombocytopenia, or anemia can occur as a result of bone marrow suppression during acute metabolic decompensations. However, these hematologic abnormalities have not been consistently reported in cohorts in the literature [<a class="bibr" href="#isovaleric-a.REF.vockley.2006.95" rid="isovaleric-a.REF.vockley.2006.95">Vockley &#x00026; Ensenauer 2006</a>, <a class="bibr" href="#isovaleric-a.REF.k_lker.2015b.1059" rid="isovaleric-a.REF.k_lker.2015b.1059">K&#x000f6;lker et al 2015b</a>].</p><p><b>Pancreatitis</b> has not been reported in individuals with classic IVA detected through NBS [<a class="bibr" href="#isovaleric-a.REF.k_lker.2015a.1041" rid="isovaleric-a.REF.k_lker.2015a.1041">K&#x000f6;lker et al 2015a</a>, <a class="bibr" href="#isovaleric-a.REF.k_lker.2015b.1059" rid="isovaleric-a.REF.k_lker.2015b.1059">K&#x000f6;lker et al 2015b</a>, <a class="bibr" href="#isovaleric-a.REF.couce.2017.355" rid="isovaleric-a.REF.couce.2017.355">Couce et al 2017</a>, <a class="bibr" href="#isovaleric-a.REF.schlune.2018.7" rid="isovaleric-a.REF.schlune.2018.7">Schlune et al 2018</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>]. Pancreatitis has been reported in some individuals with late-onset IVA and in some with early onset but delayed diagnosis and may complicate the disease course [<a class="bibr" href="#isovaleric-a.REF.dionisivici.2006.383" rid="isovaleric-a.REF.dionisivici.2006.383">Dionisi-Vici et al 2006</a>, <a class="bibr" href="#isovaleric-a.REF.k_lker.2015a.1041" rid="isovaleric-a.REF.k_lker.2015a.1041">K&#x000f6;lker et al 2015a</a>, <a class="bibr" href="#isovaleric-a.REF.k_lker.2015b.1059" rid="isovaleric-a.REF.k_lker.2015b.1059">K&#x000f6;lker et al 2015b</a>, <a class="bibr" href="#isovaleric-a.REF.schlune.2018.7" rid="isovaleric-a.REF.schlune.2018.7">Schlune et al 2018</a>].</p><p><b>Growth.</b> Information regarding growth in individuals with classic IVA is limited; growth deficiency has not been consistently reported and may vary among affected individuals. Feeding problems and gastrointestinal manifestations (e.g., vomiting) do not appear to cause growth deficiency in individuals with classic IVA [<a class="bibr" href="#isovaleric-a.REF.k_lker.2015b.1059" rid="isovaleric-a.REF.k_lker.2015b.1059">K&#x000f6;lker et al 2015b</a>], but long-term protein restriction might negatively impact growth [<a class="bibr" href="#isovaleric-a.REF.m_tze.2023a.15" rid="isovaleric-a.REF.m_tze.2023a.15">M&#x000fc;tze et al 2023a</a>].</p><p>

Although there is limited data on older individuals with classic IVA detected through NBS, there is no apparent disease progression or evidence of multisystem organ dysfunction, which is frequently observed in individuals with other organic acidemias such as propionic acidemia and methylmalonic acidemia, suggesting a more favorable prognosis in individuals with classic IVA [<a class="bibr" href="#isovaleric-a.REF.tuncel.2018.765" rid="isovaleric-a.REF.tuncel.2018.765">Tuncel et al 2018</a>].</p><p>Untreated individuals with classic IVA can develop metabolic decompensation resulting in cerebral edema and hemorrhage, coma, and death [<a class="bibr" href="#isovaleric-a.REF.vockley.2006.95" rid="isovaleric-a.REF.vockley.2006.95">Vockley &#x00026; Ensenauer 2006</a>]. Mortality is high in neonates during initial metabolic decompensation (about 30%). However, infants who survive benefit from prompt initiation of treatment, resulting in better neurocognitive outcomes compared to individuals with delayed diagnosis and treatment [<a class="bibr" href="#isovaleric-a.REF.gr_nert.2012.9" rid="isovaleric-a.REF.gr_nert.2012.9">Gr&#x000fc;nert et al 2012</a>].</p><p>Metabolic decompensations rarely occur in individuals with classic IVA after adolescence, with only a few reports of metabolic decompensation in adulthood [<a class="bibr" href="#isovaleric-a.REF.gr_nert.2012.9" rid="isovaleric-a.REF.gr_nert.2012.9">Gr&#x000fc;nert et al 2012</a>, <a class="bibr" href="#isovaleric-a.REF.tuncel.2018.765" rid="isovaleric-a.REF.tuncel.2018.765">Tuncel et al 2018</a>].</p><p><b>Classic IVA with delayed diagnosis.</b> Before the implementation of tandem mass spectrometry-based NBS that included the diagnosis of IVA, individuals with classic IVA were diagnosed at a median age of four years, most often following an acute metabolic crisis or in an individual with unexplained developmental delay [<a class="bibr" href="#isovaleric-a.REF.gr_nert.2012.9" rid="isovaleric-a.REF.gr_nert.2012.9">Gr&#x000fc;nert et al 2012</a>]. However, the majority of these individuals had preceding acute metabolic crises as neonates. Similarly, early literature originally suggested two forms of IVA, a neonatal group presenting with acute life-threatening encephalopathy and a second group presenting in childhood with nonspecific poor feeding with selective refusal of protein-rich foods, recurrent vomiting, poor weight gain, growth deficiency, intermittent ataxia, abnormal behavior, and/or neurodevelopmental delay [<a class="bibr" href="#isovaleric-a.REF.tanaka.1990.273" rid="isovaleric-a.REF.tanaka.1990.273">Tanaka 1990</a>]. Individuals who survived the first neonatal episode, however, were indistinguishable from those presenting in childhood, suggesting that there is a continuous phenotypic disease spectrum [<a class="bibr" href="#isovaleric-a.REF.vockley.2006.95" rid="isovaleric-a.REF.vockley.2006.95">Vockley &#x00026; Ensenauer 2006</a>], and delayed diagnosis may be due to missed diagnosis or later onset.</p></div><div id="isovaleric-a.GenotypePhenotype_Correlati"><h3>Genotype-Phenotype Correlations</h3><p>Limited information is available regarding genotype-phenotype correlations [<a class="bibr" href="#isovaleric-a.REF.vockley.2006.95" rid="isovaleric-a.REF.vockley.2006.95">Vockley &#x00026; Ensenauer 2006</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2023b.1063" rid="isovaleric-a.REF.m_tze.2023b.1063">M&#x000fc;tze et al 2023b</a>].</p><p>The pathogenic variant p.Ala311Val is particularly common in individuals identified through NBS and is associated with attenuated IVA in individuals who are homozygous or compound heterozygous for this variant (see <a href="#isovaleric-a.Genetically_Related_Allelic">Genetically Related Disorders</a>). These individuals exhibit only mild elevations in metabolites and have an asymptomatic disease course [<a class="bibr" href="#isovaleric-a.REF.ensenauer.2004.1136" rid="isovaleric-a.REF.ensenauer.2004.1136">Ensenauer et al 2004</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2023b.1063" rid="isovaleric-a.REF.m_tze.2023b.1063">M&#x000fc;tze et al 2023b</a>]. Additional <i>IVD</i> pathogenic variants can also result in attenuated IVA [<a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>].</p></div><div id="isovaleric-a.Nomenclature"><h3>Nomenclature</h3><p><b>Classic isovaleric acidemia/aciduria</b> refers to the presence of elevated levels of isovaleric acid in the blood/urine.</p><p><b>Isovaleryl-coenzyme A dehydrogenase deficiency</b> refers to inherited deficiency of the mitochondrial FAD-dependent enzyme isovaleryl-coenzyme A dehydrogenase resulting in isovaleric acidemia.</p></div><div id="isovaleric-a.Prevalence"><h3>Prevalence</h3><p>Prevalence of classic IVA is 1:100,000 newborns [<a class="bibr" href="#isovaleric-a.REF.moorthie.2014.889" rid="isovaleric-a.REF.moorthie.2014.889">Moorthie et al 2014</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>]. There are no known founder variants or populations with high disease prevalence.</p></div></div><div id="isovaleric-a.Genetically_Related_Allelic"><h2 id="_isovaleric-a_Genetically_Related_Allelic_">Genetically Related (Allelic) Disorders</h2><p><b>Attenuated IVA</b> (also known as "attenuated disease variant"; formerly referred to as "mild IVA"). Individuals with biallelic <i>IVD</i> variants can present with mild elevations in C5-carnitine (&#x0003c;~6 &#x003bc;mol/L on NBS) and absence of typical clinical manifestations of classic IVA (acute metabolic decompensations, seizures, motor delay, hypotonia, and movement disorder). This phenotype occurs predominantly (&#x0003e;80%) in homozygous or compound heterozygous individuals with the common <i>IVD</i> missense variant NM_002225.5:c.932C&#x0003e;T (p.Ala311Val). These individuals have normal cognitive function (mean IQ: 105&#x000b1;16), even without dietary treatment and/or medication [<a class="bibr" href="#isovaleric-a.REF.ensenauer.2004.1136" rid="isovaleric-a.REF.ensenauer.2004.1136">Ensenauer et al 2004</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>, <a class="bibr" href="#isovaleric-a.REF.m_tze.2023b.1063" rid="isovaleric-a.REF.m_tze.2023b.1063">M&#x000fc;tze et al 2023b</a>]. Infants with attenuated IVA do not need aggressive treatment and should be protected from overtreatment [<a class="bibr" href="#isovaleric-a.REF.m_tze.2021.857" rid="isovaleric-a.REF.m_tze.2021.857">M&#x000fc;tze et al 2021</a>]. These individuals can follow a normal diet without any restrictions with avoidance of prolonged and/or excessive fasting. In those with severely increased or prolonged catabolism, treatment includes intravenous glucose, rehydration with normal saline, and consideration of transient use of carnitine.</p></div><div id="isovaleric-a.Differential_Diagnosis"><h2 id="_isovaleric-a_Differential_Diagnosis_">Differential Diagnosis</h2><div id="isovaleric-a.Scenario_1_Abnormal_Newborn_1"><h3>Scenario 1: Abnormal Newborn Screening (NBS) Result</h3><div class="iconblock whole_rhythm clearfix ten_col table-wrap" id="figisovalericaTgenesofinterestinthe"><a href="/books/NBK601614/table/isovaleric-a.T.genes_of_interest_in_the/?report=objectonly" target="object" title="Table 4a. professional</i>. &#x02014;ED.</p><div id="isovaleric-a.Mode_of_Inheritance"><h3>Mode of Inheritance</h3><p>Classic isovaleric acidemia (IVA) is inherited in an autosomal recessive manner.</p></div><div id="isovaleric-a.Risk_to_Family_Members"><h3>Risk to Family Members</h3><p>
<b>Parents of a proband</b>
</p><ul><li class="half_rhythm"><div>The parents of an affected child are presumed to be heterozygous for an <i>IVD</i> pathogenic variant.</div></li><li class="half_rhythm"><div>Once a molecular diagnosis has been established in the proband, molecular genetic testing is recommended for the parents of the proband to confirm that both parents are heterozygous for an <i>IVD</i> pathogenic variant and to allow reliable recurrence risk assessment. If a pathogenic variant is detected in only one parent and parental identity testing has confirmed biological maternity and paternity, it is possible that one of the pathogenic variants identified in the proband occurred as a <i>de novo</i> event in the proband or as a postzygotic <i>de novo</i> event in a mosaic parent [<a class="bibr" href="#isovaleric-a.REF.j_nsson.2017.519" rid="isovaleric-a.REF.j_nsson.2017.519">J&#x000f3;nsson et al 2017</a>]. If the proband appears to have homozygous pathogenic variants (i.e., the same two pathogenic variants), additional possibilities to consider include:</div><ul><li class="half_rhythm"><div>A single- or multiexon deletion in the proband that was not detected by sequence analysis and that resulted in the artifactual appearance of homozygosity;</div></li><li class="half_rhythm"><div>Uniparental isodisomy for the parental chromosome with the pathogenic variant that resulted in homozygosity for the pathogenic variant in the proband.</div></li></ul></li><li class="half_rhythm"><div>Heterozygotes (carriers) are asymptomatic and are not at risk of developing the disorder.</div></li></ul><p>
<b>Sibs of a proband</b>
</p><ul><li class="half_rhythm"><div>If both parents are known to be heterozygous for an <i>IVD</i> pathogenic variant, each sib of an affected individual has at conception a 25% chance of being affected, a 50% chance of being an asymptomatic carrier, and a 25% chance of being unaffected and not a carrier.</div></li><li class="half_rhythm"><div>The neurologic phenotype of classic IVA and the frequency of acute metabolic crisis can vary widely among untreated individuals and family members with the same biallelic <i>IVD</i> pathogenic variants. This discrepancy is mostly explained by differences in the time of diagnosis as well as the start of and adherence to treatment.</div></li><li class="half_rhythm"><div>Heterozygotes (carriers) are asymptomatic and are not at risk of developing the disorder.</div></li></ul><p><b>Offspring of a proband.</b> Unless an affected individual's reproductive partner also has classic IVA or is a carrier, offspring will be obligate heterozygotes (carriers) for a pathogenic variant in <i>IVD</i>.</p><p><b>Other family members.</b> Each sib of the proband's parents is at a 50% risk of being a carrier of an <i>IVD</i> pathogenic variant.</p></div><div id="isovaleric-a.Carrier_Detection"><h3>Carrier Detection</h3><p>Carrier testing for at-risk relatives requires prior identification of the <i>IVD</i> pathogenic variants in the family.</p></div><div id="isovaleric-a.Related_Genetic_Counseling"><h3>Related Genetic Counseling Issues</h3><p>See Management, <a href="#isovaleric-a.Evaluation_of_Relatives_at">Evaluation of Relatives at Risk</a> for information on evaluating at-risk relatives for the purpose of early diagnosis and treatment.</p><p>
<b>Family planning</b>
</p><ul><li class="half_rhythm"><div>The optimal time for determination of genetic risk and discussion of the availability of prenatal/preimplantation genetic testing is before pregnancy.</div></li><li class="half_rhythm"><div>It is appropriate to offer genetic counseling (including discussion of potential risks to offspring and reproductive options) to young adults who are affected, are carriers, or are at risk of being carriers.</div></li></ul><p><b>DNA banking.</b> Because it is likely that testing methodology and our understanding of genes, pathogenic mechanisms, and diseases will improve in the future, consideration should be given to banking DNA from probands in whom a molecular diagnosis has not been confirmed (i.e., the causative pathogenic mechanism is unknown). For more information, see <a class="bibr" href="#isovaleric-a.REF.huang.2022.389" rid="isovaleric-a.REF.huang.2022.389">Huang et al [2022]</a>.</p></div><div id="isovaleric-a.Prenatal_Testing_and_Preimp"><h3>Prenatal Testing and Preimplantation Genetic Testing</h3><p>Once the <i>IVD</i> pathogenic variants have been identified in an affected family member, prenatal and preimplantation genetic testing are possible.</p><p>Differences in perspective may exist among medical professionals and within families regarding the use of prenatal testing.
