Although some begin to develop speech in the second year, others have remained nonverbal. class="inline">Clinical characteristics.</h4><p><i>WDR26</i>-related intellectual disability (ID) is characterized by developmental delay / intellectual disability, characteristic facial features, hypotonia, epilepsy, and infant feeding difficulties. To date 15 individuals, ages 24 months to 34 years, have been reported. Developmental delay is present in all individuals and ranges from mild to severe. All individuals have delayed speech. Although some begin to develop speech in the second year, others have remained nonverbal. Seizures, present in all affected individuals reported to date, can be febrile or non-febrile (tonic-clonic, absence, rolandic seizures); most seizures are self limited or respond well to standard treatment. Affected individuals are generally described as happy and socially engaging; several have stereotypies / autistic features (repetitive or rocking behavior, abnormal hand movements or posturing, and at times self-stimulation).</p></div><div><h4 class="inline">Diagnosis/testing.</h4><p>The diagnosis of <i>WDR26</i>-related ID is established in a proband with suggestive clinical features and a heterozygous pathogenic variant in <i>WDR26</i> identified by molecular genetic testing.</p></div><div><h4 class="inline">Management.</h4><p><i>Treatment of manifestations:</i> Standard treatment of developmental delay / intellectual disability, seizures, infant feeding problems, and behavioral issues.</p><p><i>Surveillance:</i> In infancy: regular assessment of swallowing, feeding, and nutritional status to determine safety of oral vs gastrostomy feeding. For all age groups: routine monitoring of developmental progress, educational needs, and behavioral issues.</p></div><div><h4 class="inline">Genetic counseling.</h4><p><i>WDR26</i>-related ID is inherited in an autosomal dominant manner. All individuals reported to date have the disorder as the result of a <i>de novo</i> pathogenic variant. If the <i>WDR26</i> pathogenic variant found in the proband cannot be detected in the leukocyte DNA of either parent, the recurrence risk to sibs is estimated to be 1% because of the theoretic possibility of parental germline mosaicism. Reference to "pathogenic variants" in this section is understood to include any likely pathogenic variants. (2) Identification of a heterozygous <i>WDR26</i> variant of uncertain significance does not establish or rule out the diagnosis.</p><p><b>Molecular genetic testing</b> in a child with developmental delay or an older individual with intellectual disability typically begins with chromosomal microarray analysis (CMA). If CMA is not diagnostic, the next step is typically either a multigene panel or comprehensive genomic testing (exome sequencing, genome sequencing). Note: Single-gene testing (sequence analysis of <i>WDR26</i>, followed by gene-targeted deletion/duplication analysis) is of unproven utility, and unless <i>WDR26</i>-related ID is strongly suspected, it is typically NOT recommended.</p><ul><li class="half_rhythm"><div class="half_rhythm"><b>Chromosomal microarray analysis (CMA</b>) using oligonucleotide or SNP arrays can identify chromosome 1q42 microdeletions that can include <i>WDR26</i>, causing <i>WDR26</i>-related ID (see <a href="/books/NBK540448/table/wdr26-id.T.molecular_genetic_testing_use/?report=objectonly" target="object" rid-ob="figobwdr26idTmoleculargenetictestinguse">Table 1</a>).</div></li><li class="half_rhythm"><div class="half_rhythm"><b>An intellectual disability (ID) multigene panel</b> that includes <i>WDR26</i> and other genes of interest (see <a href="#wdr26-id.Differential_Diagnosis">Differential Diagnosis</a>) is most likely to identify the genetic cause of the condition in a person with a nondiagnostic CMA while limiting identification of variants of uncertain significance and pathogenic variants in genes that do not explain the underlying phenotype. Note: (1) The genes included in the panel and the diagnostic sensitivity of the testing used for each gene vary by laboratory and are likely to change over time. (2) Some multigene panels may include genes not associated with the condition discussed in this <i>GeneReview</i>. (3) In some laboratories, panel options may include a custom laboratory-designed panel and/or custom phenotype-focused exome analysis that includes genes specified by the clinician. (4) Methods used in a panel may include sequence analysis, deletion/duplication analysis, and/or other non-sequencing-based tests. These 15 were identified among some 21,400 individuals with intellectual disability who had exome sequencing, giving an estimated frequency of 1:1,500 for individuals with unknown causes of intellectual disability [<a class="bibr" href="#wdr26-id.REF.skraban.2017.139" rid="wdr26-id.REF.skraban.2017.139">Skraban et al 2017</a>].</p></div></div><div id="wdr26-id.Genetically_Related_Allelic_Dis"><h2 id="_wdr26-id_Genetically_Related_Allelic_Dis_">Genetically Related (Allelic) Disorders</h2><p><b>Chromosome 1q41q42 deletions.</b> More than 20 individuals have been reported with deletions of the chromosome 1q41q42 region ranging in size from 300 Kb to 10 Mb and including <i>WDR26</i> and varied subsets of other genes [<a class="bibr" href="#wdr26-id.REF.shaffer.2007.607" rid="wdr26-id.REF.shaffer.2007.607">Shaffer et al 2007</a>]. Consistently, many individuals with 1q42 deletions demonstrate features that overlap those of individuals with intragenic pathogenic variants in <i>WDR26</i>, including facial dysmorphia, developmental delay, and a predisposition to seizures. Other clinical features seen in some individuals include cataracts, short stature, microcephaly, and multiple structural anomalies including nail hypoplasia, cleft palate, clubfoot, congenital heart disease, and congenital diaphragmatic hernia [<a class="bibr" href="#wdr26-id.REF.rice.2006.1637" rid="wdr26-id.REF.rice.2006.1637">Rice et al 2006</a>, <a class="bibr" href="#wdr26-id.REF.mazzeu.2007.1790" rid="wdr26-id.REF.mazzeu.2007.1790">Mazzeu et al 2007</a>, <a class="bibr" href="#wdr26-id.REF.shaffer.2007.607" rid="wdr26-id.REF.shaffer.2007.607">Shaffer et al 2007</a>, <a class="bibr" href="#wdr26-id.REF.slavotinek.2009.429" rid="wdr26-id.REF.slavotinek.2009.429">Slavotinek et al 2009</a>, <a class="bibr" href="#wdr26-id.REF.filges.2010.987" rid="wdr26-id.REF.filges.2010.987">Filges et al 2010</a>, <a class="bibr" href="#wdr26-id.REF.kantarci.2010.2493" rid="wdr26-id.REF.kantarci.2010.2493">Kantarci et al 2010</a>, <a class="bibr" href="#wdr26-id.REF.rosenfeld.2011.42" rid="wdr26-id.REF.rosenfeld.2011.42">Rosenfeld et al 2011</a>, <a class="bibr" href="#wdr26-id.REF.wat.2011.299" rid="wdr26-id.REF.wat.2011.299">Wat et al 2011</a>, <a class="bibr" href="#wdr26-id.REF.au.2014.441" rid="wdr26-id.REF.au.2014.441">Au et al 2014</a>, <a class="bibr" href="#wdr26-id.REF.cassina.2015.1418" rid="wdr26-id.REF.cassina.2015.1418">Cassina et al 2015</a>, <a class="bibr" href="#wdr26-id.REF.yanagishita.2019.452" rid="wdr26-id.REF.yanagishita.2019.452">Yanagishita et al 2019</a>].</p><p>Of relevance, <i>FBXO28</i>, which lies adjacent to <i>WDR26</i>, is often included in 1q41q42 chromosomal deletions. Recently, a <i>de novo FBXO28</i> frameshift variant was reported in an individual with developmental delay and seizures but absence of maxillary or gingival findings associated with mutation of <i>WDR26</i> [<a class="bibr" href="#wdr26-id.REF.balak.2018.1549" rid="wdr26-id.REF.balak.2018.1549">Balak et al 2018</a>]; this individual also had central sleep apnea and nail anomalies not noted associated with <i>WDR26</i>. These observations suggest that effects of haploinsufficiency of both <i>WDR26</i> and <i>FBXO28</i> may influence the phenotype of 1q41q42 chromosomal deletions.</p></div><div id="wdr26-id.Differential_Diagnosis"><h2 id="_wdr26-id_Differential_Diagnosis_">Differential Diagnosis</h2><p>Developmental delay with delayed speech and febrile and/or non-febrile seizures, the most frequent features of <i>WDR26</i>-related ID, are relatively common and have an extensive differential diagnosis.</p><p>The following syndromes with significant phenotypic overlap with <i>WDR26</i>-related ID have been considered in some affected individuals before the diagnosis of <i>WDR26</i>-related ID was established (<a href="/books/NBK540448/table/wdr26-id.T.disorders_with_developmental/?report=objectonly" target="object" rid-ob="figobwdr26idTdisorderswithdevelopmental">Table 2</a>).</p><div class="iconblock whole_rhythm clearfix ten_col table-wrap" id="figwdr26idTdisorderswithdevelopmental"><a href="/books/NBK540448/table/wdr26-id.T.disorders_with_developmental/?report=objectonly" target="object" title="Table 2. In the US, early intervention is a federally funded program available in all states and provides in-home services to target individual therapy needs.</p><p><b>Ages 3-5 years.</b> In the US, developmental preschool through the local public school district is recommended. Before placement, an evaluation is made to determine needed services and therapies and an individualized education plan (IEP) is developed for those who qualify based on established motor, language, social, or cognitive delay. The early intervention program typically assists with this transition. Developmental preschool is center-based; however, for children too medically unstable to attend, home-based services are provided.</p><p><b>All ages.</b> Consultation with a developmental pediatrician is recommended to ensure the involvement of appropriate community, state, and educational agencies and to support parents in maximizing quality of life. Some issues to consider:</p><ul><li class="half_rhythm"><div>IEP services:</div><ul><li class="half_rhythm"><div>An IEP provides specially designed instruction and related services to children who qualify</div></li><li class="half_rhythm"><div>IEP services will be reviewed annually to determine if any changes are needed.</div></li><li class="half_rhythm"><div>Special education law requires that children participating in an IEP be in the least restrictive environment feasible at school and included in general education as much as possible, when and where appropriate.</div></li><li class="half_rhythm"><div>Vision and hearing consultants should be a part of the child's IEP team to support access to academic material.</div></li><li class="half_rhythm"><div>PT, OT, and speech services will be provided in the IEP to the extent that the need affects the child's access to academic material. Beyond that, private supportive therapies based on the affected individual's needs may be considered. Specific recommendations regarding type of therapy can be made by a developmental pediatrician.</div></li><li class="half_rhythm"><div>As a child enters teen years, a transition plan should be discussed and incorporated in the IEP. For those receiving IEP services, the public school district is required to provide services until age 21.</div></li></ul></li><li class="half_rhythm"><div>A 504 plan (Section 504: a US federal statute that prohibits discrimination based on disability) can be considered for those who require accommodations or modifications such as front-of-class seating, assistive technology devices, classroom scribes, extra time between classes, modified assignments, and enlarged text.</div></li><li class="half_rhythm"><div>In the US:</div><ul><li class="half_rhythm"><div>Developmental Disabilities Administration (DDA) enrollment is recommended. DDA is a public agency that provides services and support to qualified individuals. Eligibility differs by state but is typically determined by diagnosis and/or associated cognitive/adaptive disabilities.</div></li><li class="half_rhythm"><div>Families with limited income and resources may also qualify for supplemental security income (SSI) for their child with a disability.</div></li></ul></li></ul></div><div id="wdr26-id.Motor_Dysfunction"><h4>Motor Dysfunction</h4><p>

<b>Gross motor dysfunction</b>
|
|
</p><ul><li class="half_rhythm"><div>Physical therapy is recommended to maximize mobility and to reduce the risk for later-onset orthopedic complications (e.g., contractures, scoliosis, hip dislocation).</div></li><li class="half_rhythm"><div>Consider use of durable medical equipment and positioning devices as needed (e.g., wheelchairs, walkers, bath chairs, orthotics, adaptive strollers).</div></li></ul><p><b>Fine motor dysfunction.</b> Occupational therapy is recommended for difficulty with fine motor skills that affect adaptive function such as feeding, grooming, dressing, and writing.</p><p><b>Oral motor dysfunction.</b> Assuming that the individual is safe to eat by mouth, feeding therapy – typically from an occupational or speech therapist – is recommended for affected individuals who have difficulty feeding as a result of poor oral motor control.</p><p><b>Communication issues.</b> Consider evaluation for alternative means of communication (e.g., <a href="https://www.asha.org/NJC/AAC/" ref="pagearea=body&targetsite=external&targetcat=link&targettype=uri">augmentative and alternative communication</a> [AAC]) for individuals who have expressive language difficulties. An AAC evaluation can be completed by a speech language pathologist who has expertise in the area. The evaluation will consider cognitive abilities and sensory impairments to determine the most appropriate form of communication. AAC devices can range from low-tech, such as picture exchange communication, to high-tech, such as voice-generating devices. Contrary to popular belief, AAC devices do not hinder verbal development of speech, but rather support optimal speech and language development.</p></div><div id="wdr26-id.SocialBehavioral_Difficulties"><h4>Social/Behavioral Difficulties</h4><p>Children may qualify for and benefit from interventions used in treatment of autism spectrum disorder, including applied behavior analysis (ABA). ABA therapy is targeted to the individual child's behavioral, social, and adaptive strengths and weaknesses and is typically performed one on one with a board-certified behavior analyst.</p><p>Consultation with a developmental pediatrician may be helpful in guiding parents through appropriate behavior management strategies or providing prescription medications, such as medication used to treat attention-deficit/hyperactivity disorder (ADHD), when necessary.</p><p>Concerns about serious aggressive or destructive behavior can be addressed by a pediatric psychiatrist.</p></div></div><div id="wdr26-id.Surveillance"><h3>Surveillance</h3><div class="iconblock whole_rhythm clearfix ten_col table-wrap" id="figwdr26idTrecommendedsurveillancefor"><a href="/books/NBK540448/table/wdr26-id.T.recommended_surveillance_for/?report=objectonly" target="object" title="Table 5. </p><ul><li class="half_rhythm"><div>All probands reported to date with <i>WDR26</i>-related ID whose parents have undergone molecular genetic testing have the disorder as a result of a <i>de novo</i>
|

<i>WDR26</i> pathogenic variant.</div></li><li class="half_rhythm"><div>Molecular genetic testing is recommended for the parents of a proband with an apparent <i>de novo</i> pathogenic variant.</div></li><li class="half_rhythm"><div>If the <i>WDR26</i> pathogenic variant found in the proband cannot be detected in the leukocyte DNA of either parent, the pathogenic variant most likely occurred <i>de novo</i> in the proband. Another possible explanation is that the proband inherited a pathogenic variant from a parent with germline mosaicism. Although theoretically possible, no instances of germline mosaicism have been reported to date.</div></li><li class="half_rhythm"><div>Theoretically, if the parent is the individual in whom the <i>WDR26</i> pathogenic variant first occurred, the parent may have somatic mosaicism for the variant and may be mildly/minimally affected.</div></li></ul><p><b>Sibs of a proband.</b> The risk to the sibs of the proband depends on the genetic status of the proband's parents: if the <i>WDR26</i> pathogenic variant found in the proband cannot be detected in the leukocyte DNA of either parent, the recurrence risk to sibs is estimated to be 1% because of the theoretic possibility of parental germline mosaicism [<a href="https://www.ncbi.nlm.nih.gov/pubmed/?term=Rahbari%20R%5BAuthor%5D&cauthor=true&cauthor_uid=26656846" ref="pagearea=body&targetsite=external&targetcat=link&targettype=uri">Rahbari</a> et al 2016].</p><p><b>Offspring of a proband.</b> Individuals with <i>WDR26</i>-related ID have not been known to reproduce; however, many are still children.</p><p><b>Other family members.</b> Given that all probands with <i>WDR26</i>-related ID reported to date have the disorder as a result of a <i>de novo</i>

<b>Family planning</b>

</p><ul><li class="half_rhythm"><div>The optimal time for determination of genetic risk and discussion of the availability of prenatal/preimplantation genetic testing is before pregnancy.</div></li><li class="half_rhythm"><div>It is appropriate to offer genetic counseling (including discussion of potential risks to offspring and reproductive options) to parents of affected individuals.</div></li></ul></div><div id="wdr26-id.Prenatal_Testing_and_Preimplant"><h3>Prenatal Testing and Preimplantation Genetic Testing</h3><p>Risk to future pregnancies is presumed to be low as the proband most likely has a <i>de novo WDR26</i> pathogenic variant. There is, however, a recurrence risk (~1%) to sibs based on the theoretic possibility of parental germline mosaicism [<a class="bibr" href="#wdr26-id.REF.rahbari.2016.126" rid="wdr26-id.REF.rahbari.2016.126">Rahbari et al 2016</a>]. However, little is known regarding how haploinsufficiency leads to specific cellular or developmental pathobiology that results in the human phenotype.</p><p><b>Gene structure.</b> The <i>WDR26</i> transcript <a href="https://www.ncbi.nlm.nih.gov/nuccore/NM_025160.6" ref="pagearea=body&targetsite=external&targetcat=link&targettype=uri">NM_025160.6</a> has 14 exons, all of which contain coding sequence.</p><p>See <a href="/books/n/gene/wdr26/?report=reader#wdr26.molgen.TA">Table A</a>, <b>Gene</b> for a detailed summary of gene and protein information.</p><p><b>Pathogenic variants.</b> The majority of <i>WDR26</i> pathogenic variants have been <i>de novo</i> frameshift or nonsense variants. However, several missense variants have been noted in exons 4 and 5. All reported pathogenic variants to date are private to individual families.</p><p>The observation that core features of the 1q41q42 deletion syndrome consistently overlap those of <i>WDR26</i>-related ID strongly supports <i>WDR26</i> haploinsufficiency as the basis of this deletion syndrome.</p><p><b>Normal gene product.</b> The WDR26 protein includes 661 amino acids, and modeling suggests that it contains 14 variably perfect WD40 repeats. Together the 14 WD40 repeats compose two seven-bladed β propeller structures [<a class="bibr" href="#wdr26-id.REF.skraban.2017.139" rid="wdr26-id.REF.skraban.2017.139">Skraban et al 2017</a>]:</p><ul><li class="half_rhythm"><div>Domains WD1-WD7 (amino acids 1-353) comprise an N-terminal β propeller.</div></li><li class="half_rhythm"><div>Domains WD8-WD14 (amino acids 354-645) comprise a C-terminal β propeller, which includes the conserved LisH (LIS1 homology) and CTLH (C-terminal LIS homology) domains.</div></li></ul><p>Although specific cellular functions for WDR26 are unclear, many WD40 repeat proteins play key roles in cellular scaffolding [<a class="bibr" href="#wdr26-id.REF.zhu.2004.579" rid="wdr26-id.REF.zhu.2004.579">Zhu et al 2004</a>, <a class="bibr" href="#wdr26-id.REF.stirnimann.2010.565" rid="wdr26-id.REF.stirnimann.2010.565">Stirnimann et al 2010</a>, <a class="bibr" href="#wdr26-id.REF.sun.2013.16715" rid="wdr26-id.REF.sun.2013.16715">Sun et al 2013</a>].</p><p><b>Abnormal gene product.</b> Most pathogenic <i>WDR26</i> variants result in haploinsufficiency caused by either loss-of-function variants or deletion of part or all of the gene.
