EFFECTIVE DATE: The revised Application for a Ruth L. Kirschstein
Individual National Research Service Award (PHS 416-1) and Progress
Report for Continuation Support (PHS 416-9) are now available and may
be used by applicants. For the November 15 and December 5, 2002
application receipt dates, both the previous versions (Rev. 12/98) and
the new versions (Rev. 6/02) will be accepted. Individuals who have
initiated their applications using the 12/98 forms are encouraged to
continue with this form rather than moving to the new version. For the
April 2003 application receipt date and beyond, use of the new version
(Rev. 6/02) is required.
As in the past, applicants should read the instructions carefully. The
following is a list of notable changes:
PHS 416-1:
o The name has been changed to the Ruth L. Kirschstein Individual
National Research Service Award.
o The PHS 416-1 will only be available electronically.

o Forms have been modified to enable the fields to be filled in
directly using the Adobe Acrobat Reader Software.
o Human Subjects Research: Specific instructions are provided for the
use of human subjects in research, including new format pages for Women
and Minority Inclusion enrollment.
o Vertebrate Animals: Institutional Animal Care and Use Committee
(IACUC) approval of proposed research involving vertebrate animals is
no longer required before an application will be accepted for peer
review. As part of the peer review process, the peer review group will
continue to address the adequacy of animal usage and protections in
applications; however, IACUC approval is required only on a "just-in-
time" basis before an award can be made. See the August 8, 2003 NIH
Guide Notice (OD-02-064) for more details on this policy change. It

o Face Page: Form Page 1:
Human Subjects Research box (Item 9) has been modified as follows:
9a: No/Yes check applicable box for "Research Exempt."
9b: If 9a is checked "No," provide Human Subjects Assurance Number.
9c: check applicable box for NIH Defined Phase III Clinical Trial.
Vertebrate Animals box (Item 10) has been modified as follows:
10: No/Yes check applicable box and, if "yes" is checked, provide
Animal Welfare Assurance No.
o Personal Data Page:
The Personal Data on Individual NRSA Fellowship Applicant Page has been
modified to reflect updated Race and Ethnicity Categories.
o New Format Pages

The forms will also be available in Rich Text Format (RTF), a
standardized way to encode various text formatting properties, such as
bold characters and typefaces, as well as document formatting and
structures. RTF is supported across a number of software and hardware
platforms. RTF files can be opened in many word processors and other
RTF-aware software packages with much of the formatting left intact.
In addition, many word processing programs will also allow users to
save a document in RTF when you select "Save As "
NOTE: Other software packages for completing these applications may be
available from other sources; however, it is essential that the type
size and format specifications are met or the application will be
returned without review.
