<p>This Notice of Funding Opportunity (NOFO) will support analyses of existing RADx-UP data by proposing novel questions related to SARS-CoV-2 testing and health disparities. These projects will explore important questions focused on COVID-19 testing access and uptake, factors affecting testing and related COVID-19 outcomes, and meta-analyses of specific COVID-19 response and intervention approaches within and across populations.</p><p>The funding for this program is provided from the American Rescue Plan Act of 2021, Public Law 117-2. </p>

<p>This Notice of Funding Opportunity (NOFO) supports Phase IV of the <a href="https://www.nih.gov/research-training/medical-research-initiatives/radx/radx-programs#radx-up">Rapid Acceleration of Diagnostics Underserved Populations (RADx -UP) initiative</a>. This Notice of Funding Opportunity highlights the need to leverage existing data regarding SARS-CoV-2 testing to understand and address COVID-19 morbidity and mortality disparities among underserved and vulnerable populations across the United States.</p><p><strong>Key Definitions</strong></p><p>This NOFO is applicable to those populations that are underserved as well as populations that are COVID-19 vulnerable due to medical, geographic, and social factors, as defined below (referred to as underserved and vulnerable elsewhere in this NOFO):</p><p><i>Underserved</i><strong>:</strong> NIH-designated populations with health disparities and/or other groups known to experience barriers to accessing needed health care services or have inadequate health care coverage. A full description can be found at <a href="https://www.nimhd.nih.gov/about/overview/">https://www.nimhd.nih.gov/about/overview/</a>.</p><p><i>COVID-19 medically and/or socially vulnerable populations</i><strong>:</strong> Residents of nursing homes and assisted living facilities; community-dwelling older adults; individuals with intellectual, developmental, sensory, or physical disabilities, cognitive impairment or dementia, or communication disorders; homeless populations; individuals involved with the criminal or juvenile justice systems (incarcerated or under community supervision); individuals with medical comorbidities known to increase risk of severe COVID-19, including heart failure and related cardiovascular conditions, diabetes mellitus, chronic lung disease, obesity, HIV/AIDS; pregnant and post-partum women; children and adolescents; individuals living in congregate housing such as shelters or residential treatment facilities; individuals in overcrowded housing; individuals with substance use disorders or serious mental illness; migrant and immigrant populations; residents of tribal lands or reservations; communities exposed to high rates of air pollution or other toxic exposures; and rural and remote communities.</p><p><i>SARS-CoV-2 Testing (also referred to as COVID-19 Testing ): </i>Includes access to FDA-authorized/cleared/approved test kits and related supplies for polymerase chain reaction (PCR) and rapid (molecular or antigen) tests used to <a href="https://www.fda.gov/medical-devices/coronavirus-disease-2019-covid-19-emergency-use-authorizations-medical-devices/in-vitro-diagnostics-euas">diagnose</a> infection. PCR tests must be assayed in Clinical Laboratory Improvement Amendments (CLIA) certified laboratories (e.g., hospital, public health, or commercial) to administer the tests and return test results to participants as quickly as possible. Rapid tests are conducted at or near the place where a specimen is collected, usually outside a laboratory setting and provide results to patients within minutes. The proposed (and implemented) COVID-19 testing must be fully FDA-Emergency Use Authorized/cleared/approved, and used solely for the specific, on-label purpose for which they were developed and authorized/approved/cleared. This includes sample collection, assays, and test performance. In the context of COVID-19, these tests are diagnostic tests that provide results to patients within minutes.</p><p><i>Point-of-Care Testing</i><strong>:</strong> testing done at or near the point of care that involves performing a diagnostic test.</p><p style="margin-left:0in;"><strong>Background</strong></p><p>The <a href="https://www.nih.gov/research-training/medical-research-initiatives/radx/radx-programs#radx-up">RADx -UP initiative</a> seeks to address COVID-19 morbidity and mortality disparities among underserved and vulnerable populations with disproportionate rates of SARS-CoV-2 and/or undue COVID-19 burden by understanding strategies and interventions to increase testing access, acceptability, and uptake. To date, more than 135 projects are engaging participants in all US states as well as American Samoa, the District of Columbia, Guam, the Northern Mariana Islands, Puerto Rico, and the US Virgin Islands.</p><p><strong>NOFO Goals</strong></p><p>The Office of the Director (OD) is requesting applications for grants (R21) addressing the objectives described below. Projects supported via this Notice of Funding Opportunity will be part of the RADx -UP initiative. This Novel Insights through Cross-Site Analyses of Existing RADx-UP Data opportunity will support new exploratory/developmental grants for analysis of existing data collected within the RADx-UP consortium. Successful applications will leverage common demographics and other data from a minimum of three sites (with geographic diversity; inclusion of a greater number of sites is encouraged) to ask important questions around COVID-19 testing access and uptake. Applications that address factors affecting COVID-19 testing and related COVID-19 outcomes, specific COVID-19 response, intervention approaches within and across populations, and other novel questions are encouraged.</p><p>To maximize the scientific potential of valuable RADx-UP data and resources, analyses of existing archived data will allow applicants to further develop and build upon original research. Analyses of existing datasets offers a cost-effective way to collaborate and foster research within NIH’s scientific mission. The application of rigorous and new analysis methodologies that merge or link various RADx-UP datasets and de-identified variables is encouraged. Generation of metadata will allow researchers the opportunity to increase statistical power and accuracy in estimates across studies, going beyond original hypotheses to answer new research questions. </p><p><strong>Research Objectives:</strong></p><p>This research seeks to gain novel insights regarding data collected by RADx-UP studies that have shared both common and project-defined data elements on multiple topics related to COVID-19 testing and disparities among underserved and vulnerable groups.</p><p>All applications utilizing existing RADx-UP datasets and collections to explore new research questions relevant to the mission of RADx-UP will be considered. Generation of new primary data is not allowed. This Notice of Funding Opportunity will not support the collection of new data. </p><p>The data analyses will be focused on underserved and vulnerable populations and questions related to reducing health disparities based on topics and research studies that <i>include, but are not limited to</i>, the following areas:</p><ul><li><i>Testing Interventions:</i> Analyses to evaluate the effectiveness of testing interventions and/or what testing interventions can be implemented to decrease COVID-19 disparities in different regions of the U.S. and across demographic groups.</li><li><i>COVID-19 Testing Options:</i> Analyses to determine what COVID-19 testing options (e.g., surveillance, symptomatic, etc.) are most acceptable and in what setting (such as community-based organizations, health clinics, school/academic environments).</li><li><i>Common Data Elements:</i> Analyses of Common Data Elements for novel data comparison and synthesis across project sites and smaller population groups. RADx-UP CDEs collected from the projects are specified at<a href=" https://radx-up.org/research/cdes/"> https://radx-up.org/research/cdes/</a>. </li><li><i>Social Determinants of Health (SDOH): </i>Analyses on how SDOH help identify, understand, and address testing and vaccine access and uptake in low-resourced geographic areas or communities with high levels of social vulnerability. </li><li><i>Community-Engaged Interventions to address Stigma, Bias, Distrust and Fear: </i>Analyses on what type of community-driven interventions effectively ameliorate stigma, bias, distrust, and fear regarding symptomatic and asymptomatic COVID-19 testing and vaccination. </li><li><i>Psychosocial, Behavioral, and Ethical Factors Driving COVID-19 Testing and Vaccination:</i> Analyses on what specific psychosocial, behavioral, and ethical factors drive COVID-19 testing and vaccine uptake within and across underserved populations, including the role of healthcare, academic, and social service settings in response to COVID-19 health disparities. </li><li><i>Messaging and Policies Impact on COVID-19 Testing and Vaccination:</i> Analyses on whether and how messaging and policies at the local, state, and federal levels in different jurisdictions affect COVID-19 testing and vaccine uptake over time.</li></ul><p><strong>Data Sources</strong></p><p>The "Novel Insights through Cross-Site Analyses of Existing RADx-UP Data" Projects will build upon the work of RADx-UP and other <a href="https://www.nih.gov/research-training/medical-research-initiatives/radx/radx-programs">RADx</a> (<a href="https://www.nih.gov/research-training/medical-research-initiatives/radx/radx-programs">https://www.nih.gov/research-training/medical-research-initiatives/radx/radx-programs</a>) focused projects.</p><p>The <a href="https://radx-hub.nih.gov/">RADx Data Hub</a> (<a href="https://radx-hub.nih.gov/">https://radx-hub.nih.gov/</a>) will serve as a primary resource for deidentified data from the RADx-UP projects. To effectively use these data, applicants are encouraged to collaborate with investigators and community partners using innovative statistical strategies.</p><p>Single or multiple data sets may be utilized to develop and test new analytic approaches.</p><p>Key project considerations are listed below:</p><p style="margin-left:0in;">Projects awarded under this NOFO will be expected to work collaboratively with each other and with other projects related to COVID-19 testing research.</p><p>RADx -UP Phase IV projects must demonstrate relevance of the scientific questions to COVID-19 testing among underserved and COVID-19 vulnerable populations. Projects should utilize rigorous analyses and methodology. Additional information is available at <a href="https://researchmethodsresources.nih.gov/">https://researchmethodsresources.nih.gov/</a>.</p><p style="margin-left:0in;">Results from these projects must be disseminated rapidly to increase COVID-19 prevention and control in communities that have been impacted disproportionately. Use of the <a href="https://www.ncbi.nlm.nih.gov/pmc/about/nihpreprints/">NIH Preprint Pilot</a> is encouraged.</p><p><strong>Data Collection, Sharing, and Coordination:</strong></p><p>All projects funded by this NOFO must:</p><p style="margin-left:0in;">Align with all requirements of new NIH Data Management and Sharing (DMS) policy to promote the sharing of scientific data. <a href="https://sharing.nih.gov/data-management-and-sharing-policy/planning-and-budgeting-for-data-management-and-sharing/writing-a-data-management-and-sharing-plan#after">https://sharing.nih.gov/data-management-and-sharing-policy/planning-and-budgeting-for-data-management-and-sharing/writing-a-data-management-and-sharing-plan#after</a></p><p><strong>Research Plan:</strong></p><p>Projects must include an evaluation plan demonstrating how the proposed analyses will be assessed for integrity, usefulness, and impact.</p><p>Applications must include milestones towards progress and a timeline for completion.</p><p>Recipients are expected to demonstrate knowledge of and to comply with federal, state, local, and/or Tribal requirements on testing, reporting, and surveillance policies in study protocols.</p><p>Recipients must provide letters of support from any newly engaged community partners and should include community partners (where possible) as investigators. Budgets should reflect active participation by community partners to the extent possible. When required, Tribal resolutions should be included with the application, if possible, but before funds are awarded in all cases.</p><p>Applications Not Responsive to this NOFO. The following types of projects will be considered non-responsive and will not be reviewed:</p><ul><li>Projects without a focus on one or more underserved and COVID-19 vulnerable populations</li><li>Projects that do not propose use of data from at least three RADx-UP project sites</li><li>Projects that do not demonstrate relevance of the scientific questions to COVID-19 testing among underserved and COVID-19 vulnerable populations</li><li>Projects that involve COVID-19 testing interventions outside the United States</li><li>Projects that do not have an infrastructure to rapidly report study findings and impact</li></ul><p>Investigators planning to apply in response to this NOFO are encouraged to contact and discuss their proposed research/aims with Program staff listed on this NOFO well in advance of the application receipt date to better determine appropriateness and interest. <p class="Normal">Applicant organizations must complete and maintain the following registrations as described in the SF 424 (R&R) Application Guide to be eligible to apply for or receive an award. All registrations must be completed prior to the application being submitted. Registration can take 6 weeks or more, so applicants should begin the registration process as soon as possible. The <a href="https://grants.nih.gov/grants/policy/nihgps/HTML5/section_2/2.3.9_application_receipt_information_and_deadlines.htm#Electron:~:text=by%22%20dates%20apply.-,2.3.9.2%20Electronically%20Submitted%20Applications,-For%20applications%20submitted">NIH Policy on Late Submission of Grant Applications</a> states that failure to complete registrations in advance of a due date is not a valid reason for a late submission.</p>

SAM registration includes the assignment of a Commercial and Government Entity (CAGE) Code for domestic organizations which have not already been assigned a CAGE Code.

NIH NOFOs outline intended research goals and objectives. Post award, NIH will review and measure performance based on the details and outcomes that are shared within the RPPR, as described at 45 CFR Part 75.301 and 2 CFR Part 200.301.</p><p>The Federal Funding Accountability and Transparency Act of 2006 (Transparency Act), includes a requirement for recipients of Federal grants to report information about first-tier subawards and executive compensation under Federal assistance awards issued in FY2011 or later. All recipients of applicable NIH grants and cooperative agreements are required to report to the Federal Subaward Reporting System (FSRS) available at <a href="//grants.nih.gov/grants/guide/url_redirect.php?id=11170">www.fsrs.gov</a> on all subawards over $25,000. See the <a href="https://grants.nih.gov/grants/policy/nihgps/HTML5/section_4/4.1.8_federal_funding_accountability_and_transparency_act__ffata_.htm">NIH Grants Policy Statement</a> for additional information on this reporting requirement.</p><p>In accordance with the regulatory requirements provided at 45 CFR 75.113 and 2 CFR Part 200.113 and Appendix XII to 45 CFR Part 75 and 2 CFR Part 200, recipients that have currently active Federal grants, cooperative agreements, and procurement contracts from all Federal awarding agencies with a cumulative total value greater than $10,000,000 for any period of time during the period of performance of a Federal award, must report and maintain the currency of information reported in the System for Award Management (SAM) about civil, criminal, and administrative proceedings in connection with the award or performance of a Federal award that reached final disposition within the most recent five-year period. The recipient must also make semiannual disclosures regarding such proceedings. Proceedings information will be made publicly available in the designated integrity and performance system (currently FAPIIS). This is a statutory requirement under section 872 of Public Law 110-417, as amended (41 U.S.C. 2313).
