<p class="regulartext">NIH announces the availability of three revised (9/2004) forms: <br>
|
|
1) PHS 2271 (Statement of Appointment); <br>
|
|
2) PHS 3734 (Official Statement Relinquishing Interest and Rights in a PHS Research Grant); <br>
|
|
3) HHS 568 (Final Invention Statement and Certification). </p>
|
|
<p class="regulartext">The newly revised forms and instructions are available at <a href="//grants.nih.gov/grants/forms.htm">http://grants.nih.gov/grants/forms.htm </a>. The forms continue to be provided in two electronic formats MS Word and PDF-fillable using Adobe Acrobat Reader Software. Free Adobe Software may be accessed at <a href="http://www.adobe.com/products/acrobat/readstep.html" Title="Link to Non-U.S. Government Site">http://www.adobe.com/products/acrobat/readstep.html </a>. </p>

<p class="regulartext"><strong>PHS 2271 (Statement of Appointment) </strong></p>
|
|
<p class="regulartext">There is only one notable change on this form. Trainees are now requested to provide only the last four digits of the Social Security Number. While providing this information remains voluntary, it is hoped that by limiting the data to only the last four digits, individuals will be more receptive to providing it. This vital information continues to provide the agency with accurate identification of individuals and for management of PHS grant programs. </p>

<p class="regulartext">Effective date: The Statement of Appointment Form (revised 9/2004) may be used immediately and MUST be used for all trainee appointments made on/after May 1, 2005. </p>
|
|
<p class="regulartext"><strong>PHS 3734 (Official Statement Relinquishing Interest and Rights in a PHS Research Grant) </strong></p>
|
|
<p class="regulartext">There are no changes to the data elements or instructions of the PHS 3734 at this time. </p>
|
|
<p class="regulartext">Effective date: This version (09/2004) is available immediately. </p>
|
|
<p class="regulartext"><strong>HHS 568 (Final Invention Statement and Certification) </strong></p>
|
|
<p class="regulartext">There is only one notable change. Awardees are instructed to submit only the original. An additional copy is no longer required. </p>
|
|
<p class="regulartext">Effective date: This version (09/2004) is available immediately. </p>
|
|
