People with a usual source of care whose health provider usually asks about prescription medications and treatments from other doctors

Numerator

Subset of the denominator who answered "Yes" to the question identified in the denominator

Comments

Usual source of care is defined as a particular doctor's office, clinic, health center, or other health care facility to which an individual usually would go to obtain health care service.

This measure is based on responses to the CHIS adult measure AJ80, teen measure TI17, and child measure CD36 which asked respondents, "Is there anyone at your doctor's office or clinic who helps coordinate your care with other doctors or services such as tests or treatments?"

The 2011 AHA IT Supplement was sent to 6,253 non-Federal acute care hospitals in the United States, including non-AHA member hospitals. The response rate was 54.8%. If data are missing, the hospital is not included in the numerator or denominator of measures.
