Figure 4 provides a detailed illustration the patient flow plan for a smallpox clinic. The flow starts with "Greeting/Entry Screening." The suggested announcement is, "Anyone not feeling well [or who thinks they may have come in contact with someone with smallpox,] come this way, please." Patients go either to "Form Distribution," or to "Post-Event Precaution Area".

Patients who go to "Form Distribution" proceed to "Briefing, Consent," then to "Triage." "Triage" flows to either "Testing," "Witnessed Consent and Vaccination," or "Contraindication or Decline." All flow then to "Form Collection and Exit Counseling," then "Home." At any step along this stream, patients may be diverted to "Crisis Counseling," then re-enter the main stream at the point at which they exited.

Patients who proceed to "Post-Event Precaution Area" are first outfitted with an N95 mask. The first point is "Medical/Contact Evaluation." From there, patients flow either to "Not Suspect Case or Contact," "Critical," or "Post-Event Component: Suspect Cases and Contacts."

From "Not Suspect Case or Contact," the flow proceeds to "Form Distribution," "Briefing, Consent," then "Testing." Patients proceed either to "Witnessed Consent and Vaccination," or to "Contraindication or Decline." All flow to "Form Collection and Exit Counseling"—where they may flow to "Crisis Counseling"—then "Home."

From "Critical," the flow is directly to "Transport to Medical Care Facility."

From "Post-Event Component: Suspect Cases and Contacts," the flow proceeds to "Form Distribution," "Suspect Briefing, Consent," then to either "Decline Vaccination," or "Witnessed Consent and Vaccination." "Decline Vaccination" flows to "Isolation Counseling," then to "Isolation Facility." "Witnessed Consent and Vaccination" flows to "Form Collection and Exit Counseling," then to "Home."
